The Medical Records Technician / Release of Information Clerk is coordinates the maintenance of medical record files (acute, residential, and outpatient), provides telephone and reception coverage for the Health Information Services (HIS) Department, coordinates processing of discharge and active charges, maintains accurate computerized databases, medical transcription and decrypting, processes daily census reports, handles Release of Information (ROI) requests, and other duties to assist in the efficient functioning of the HIS Department. Additionally, the Medical Records Technician:
Abides by the system's policies and procedures, State and Federal regulations, and HIPAA guidelines while processing patient's ROI requests.
Ensures proper patient/parent/guardian consent forms have been documented.
Coordinates all requests for ROI requests; researches and investigates all health record release of information requests that are submitted to the department; monitors phone calls in reference to ROI questions; reviews records to determine if a physician clearance approval should be obtained for the ROI.
Maintains computerized tracking system of all ROI's; processes ROI in a timely manner and forwarding data to the next provider.
Processes discharged records; enters and maintains computerized deficiency system and prints weekly reports; analyzes each record, tags and untags deficiencies.
Assembles charges upon discharge for retrospective and discharge processing; copies records for Retro-Insurance Reviews.
Assists in verification of billing ICD-9 codes as needed; maintains weekly billing Readiness Report.
Enters patient demographics and quality assessment data into HIS Department database on a daily basis.
Retrieves and files reports or transcription on a daily basis; retrieves records for direct caregivers to complete or review; purges records as necessary.
Acts as a receptionist for facility HIS department.
Decrypts, corrects, and prints transcribed reports, tags reports with appropriate physician's tag, and places in appropriate medical record; accurately logs all reports received into transcription log daily.
Maintains permanent files ensuring that records are filed in appropriate slots.
Evaluates supply stores weekly and prepares requisitions for new supplies as needed.
