The Palliative Care APRN Program Director is an employed clinician who works in collaboration with the Medical Director, Palliative Care Programs, to oversee team APRNs, SWs, Chaplains and Nurses and to manage the daily operations of the programs. Also responsible to achieve and maintain program certifications, as well as assist in the development of partnerships with external providers & entities. This part of the role is expected to occupy at least 50% of available time. Working in collaboration with the Medical Director, Palliative Care Programs, or an IDT physician, acts as a team member for palliative care patients in the Community-Based Outpatient Palliative Care Program. In this part of the role – which is expected to occupy the other nearly 50% of available time – the APRN is responsible for evaluating and coordinating patient/family needs to enable preventative and supportive care. He/she may provide clinical services, symptom management, consultation or perform an assessment related to longing eligibility for services, in addition to providing evaluation of disease progression. The APRN utilizes assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life for the patient/family unit. The APRN collaborates with the IDT and the palliative care physician to assure that the physical, emotional, and spiritual needs of patient/family are met.

Other requirements of this role include: • Education: Master’s Degree in Nursing or a related field. Additional MBA preferred. • Licensure: Current Florida license as an Advanced Registered Nurse Practitioner. Current DEA (Drug Enforcement Agency) license. • Certifications: CPR required, Advanced Certified Hospice and Palliative Nurse (ACHPN®) preferred • NPI provider and ability to bill CMS (Medicare & Medicaid) services or ability to obtain same. • Experience in developing and the daily management of palliative care programs required. • Minimum of two (2) years nursing experience required. • Minimum of two (2) years experience as an APRN in palliative care required. • Coding & documentation experience required and expertise preferred. • Experience with pain and symptom management is highly preferred • Strong communication and assessment skills • Valid Florida driver’s license and automobile insurance as per agency policies • Ability to make clinic/home/hospital/facility visits • Basic keyboarding and computer skills including Microsoft Outlook, Word or comparable programs
