Vanderbilt University Medical Center (VUMC) is accepting applications for Case Managers within our Transition Management Office (TMO). The TMO administrative office is located in Medical Center North, on campus serving VUMC. The initial criteria for RN Care Manager are a Bachelor of Science in Nursing (BSN) and a minimum of three years of nursing experience as a Registered Nurse . As a RN Case Manager , you will be expected to use critical thinking skills, based on your nursing assessment, while working with the clinical care team, to ensure progression of the patient's plan of care through transition. This role has accountability for a patient's reasonable, and appropriate transition through the continuum of care, assisting patients and families in establishing patient specific long-term and short-term goals, removing barriers and appropriately manage efficient and effective use of resources. The RN Case Manager provides leadership to the health care team to coordinate patient and family interventions and is the catalyst that identifies indicators for post-acute care utilization, referring patients (within the designated patient population) to the appropriate next level or site of care. The RN Case Manager serves in an advocacy role on behalf of patients, families, and caregivers to ensure equitable healthcare provision and assists in developing and meeting established outcomes for patients, as well as institutional goals, which among others will include customer satisfaction, and institutional financial objectives.

Bachelor of Science in Nursing
BSN Graduate of an approved/accredited nursing program; MSN preferred
Three years of RN experience or the equivalent required; prefer 4-7 years
Inpatient hospital experience is required
Prior experience in case management preferred
Organizational skills and the ability to work independently; must be a critical thinker and problem solver
Strong interpersonal and leadership skills in dealing with healthcare providers, patients, and internal staff members
Strong organizational skills, attention to detail, and a positive attitude are vital
RN license (TN)
Three years of nursing or case management experience

RN Sign-On Bonus Eligibility Requirements :
Registered Nurses who have their BSN or MSN degree and at least six months of RN experience may be eligible for the sign-on bonus for certain departments.
The sign on bonus is not applicable to current VUMC staff or former staff who have worked for VUMC in the past two years.
