Provides administrative case management including more than 600 mental health consumers. Responsibilities include annual contact and assessments with consumers, case planning, service linkage, and client advocacy.
MAJOR DUTIES AND RESPONSIBILITIES:
Completes all paper work accurately and on time in order to assure timely billing and to meet the Counties paperwork standards. Maintains Base Service Unit (BSU) caseload assuring that all necessary paperwork iscompleted and sent to the county program office in a timely manner. Monitors the counties Outpatient Commitments and schedules conferences and hearings as needed. Participates in the scheduled conference or hearing presenting testimony in reference to the consumer's status. Works with each consumer to assure ongoing assessment of functioning level, monitor behavior, link to appropriate services and support systems for either fiscal entitlements and treatment services, plan for the best possible life experience, and advocate for the consumers' best interest in all areas of their life. Provides clinical service continuity with mental health service providers to assure accurate information sharing between providers as relative to BSU case manager issues. Arranges face to face contact with each consumer open in the BSU annually to re-determine liability and assess, plan and refer to treatment services and other support systems if indicated. Works with business office to coordinate commercial insurance issues, liabilities and county funding as needed. Coordinates referrals, transfers of clients to appropriate services.
